Title: Housing Navigator

Job Type: Full time

Salary: $55,000

Reports to: Director of Client Services

Work Hours: Office hours are between 9AM-6PM, Monday-Friday. Schedule will vary between these hours to meet program needs.

The Haven Project’s Mission is to equip and empower homeless unaccompanied young adults 17-24 with the skills and support they need to be successful. Intensive case management, vocational training and educational interventions are key components of our strategy to address the needs of these high-risk homeless young adults.

Position Description:

The Housing Navigator provides assessment, housing focused case management, and community referrals for housing unstable youth, ages 17-24. This position will help housing unstable young adults in transitional housing and rapid rehousing. They are responsible for providing assistance and support to homeless individuals in housing readiness, accessing housing assistance programs (e.g. Rapid Rehousing) and permanent housing placements. This position builds and maintains positive relationships with local landlords and property managers on the North Shore. This position requires a high degree of flexibility in responding to emergent needs of young people in crisis as well as good organizational and documentation skills.

Work to take place in an office setting, community agencies, and outdoors as needed. Will require local travel. Duties include Coordinated System navigation, maintenance of thorough and accurate records in both our internal ETO system and in Homeless Management Information System (HMIS). The worker is not substantially exposed to adverse environmental conditions. Occasionally transports materials weighing up to 50 pounds. Frequently ascends/descends stairs in buildings without elevators

Essential Duties and Responsibilities:

  • Provide assessment, case management, housing navigation, and referral services to homeless youth ages 17-24 placed in the transitional housing/rapid rehousing program;
  • Conduct Youth Coordinated Entry intake and assessment and screening
  • Create individual service & transition plan with youth including independent living skills and income stability
  • Case coordination and advocacy within agency and with other community partners
  • Maintain detailed participant records, goal oriented service plans and prepare required monthly reports regarding services provided
  • As directed, contact youth via phone and social media
  • Maintain good communications with collateral services providers and family members.
  • Ensure the safety of program participants by working with the team to enforce program safety protocols.
  • Document program activities as needed for program evaluation
  • Attend staff trainings and meetings as required.
  • Flexible hours to meet the needs of the program
  • Present themselves professionally and positively at all times
  • Facilitate group activities for the youth involved in the program in coordination with the other project staff
  • Provide participant with listings from housing location team, housing authority, internet (e.g. Craigslist), and internal database of landlords/management firms/owners.
  • Provide information and instruction to participants regarding how to complete a housing application, housing search, tenant rights and responsibilities, including observation of rental agreement rules, and being a good neighbor.
  • Ensure participants have the necessary items to secure housing
  • Assist participants with completing housing applications and securing housing. Transport participants to housing and job searches and occasional visits to relevant social service agencies.
  • Maintain on-going relationships with landlords and other housing providers, including acting as a liaison between landlord and client, as needed.

Qualifications:

  • Bachelor’s degree in related field
  • Bilingual – Spanish/English required
  • 2-3 yrs. experience working with at risk populations required
  • Knowledge of housing assistance programs
  • Passion for ending youth homelessness
  • Maintaining a non-judgmental attitude and a display of unconditional positive regard when working with persons experiencing homelessness
  • Ability to work respectfully with diverse populations
  • Creative and critical solution based thinking
  • Excellent computer skills required
  • Must be able to perform extensive charting, electronic data entry and documentation
  • Strong advocacy skills
  • Excellent communication skills
  • Personal transportation required

Evaluation:

  • This position will report to and be evaluated by the Director of Client Services
